Mark Andrews 0854f63114 Remove INSIST from from new_reference 
RBTDB node can now appear on the deadnodes lists following the changes
to decrement_reference in 176b23b6cd to
defer checking of node->down when the tree write lock is not held.  The
node should be unlinked instead.
