dd2a8c8f72 mlx4core: Use-after-free causes a resource leak in flow-steering detach
mlx4_QP_FLOW_STEERING_DETACH_wrapper first removes the steering
rule (which results in freeing the rule structure), and then
references a field in this struct (the qp number) when releasing the
busy-status on the rule's qp.

Since this memory was freed, it could reallocated and changed.
Therefore, the qp number in the struct may be incorrect,
so that we are releasing the incorrect qp. This leaves the rule's qp
in the busy state (and could possibly release an incorrect qp as well).

Fix this by saving the qp number in a local variable, for use after
removing the steering rule.

f29c9901a4 mlx4core: Fix a memory leak when deleting slave's resources
mlx4_delete_all_resources_for_slave() in the resource tracker should free
all memory allocated for a slave. While releasing memory of fs_rule,
it misses releasing memory of fs_rule->mirr_mbox.

527762b2f7 mlx4: Fix a memory leak bug.
In function mlx4_opreq_action(), pointer "mailbox" is not released,
when mlx4_cmd_box() return and error, causing a memory leak bug.
Fix this issue by going to "out" label, mlx4_free_cmd_mailbox() can
free this pointer.

c8301cbb0f mlx4: Map core_clock page to user space only when allowed
Currently when we map the hca_core_clock page to the user space,
there are vulnerable registers, one of which is semaphore, on
this page as well. If user read the wrong offset, it can modify the
above semaphore and hang the device.

Hence, mapping the hca_core_clock page to the user space only when
user required it specifically.

After this patch, mlx4 core_clock won't be mapped to user space by
default. Oppose to current state, where mlx4 core_clock is always mapped
to user space.

29ed53850e mlx4, mthca: Silence warnings about no-op alignment operations
Since commit 8fa6abb6f4 ("Expose clang's alignment builtins and use
them for roundup2/rounddown2"), clang emits warnings for several
alignment operations in these drivers because the operation is a no-op.
The compiler is arguably being too strict here, but in the meantime
let's silence the warnings by conditionally compiling the alignment
operations.

105a37cac7 linuxkpi: Add more pci functions needed by DRM
-pci_get_class : This function search for a matching pci device based on
   the class/subclass and returns a newly created pci_dev.
 - pci_{save,restore}_state : This is analogous to ours with the same name
 - pci_is_root_bus : Return true if this is the root bus
 - pci_get_domain_bus_and_slot : This function search for a matching pci
   device based on domain, bus and slot/function concat into a single
   unsigned int (devfn) and returns a newly created pci_dev
 - pci_bus_{read,write}_config* : Read/Write to the config space.

While here add some helper function to alloc and fill the pci_dev struct.

1866c98e64 Infiniband clients must be attached and detached in a specific order in ibcore.
Currently the linking order of the infiniband, IB, modules decide in which
order the clients are attached and detached. For example one IB client may
use resources from another IB client. This can lead to a potential deadlock
at shutdown. For example if the ipoib is unregistered after the ib_multicast
client is detached, then if ipoib is using multicast addresses a deadlock may
happen, because ib_multicast will wait for all its resources to be freed before
returning from the remove method.

Fix this by using module_xxx_order() instead of module_xxx().

7029da5c36 Mark more nodes as CTLFLAG_MPSAFE or CTLFLAG_NEEDGIANT (17 of many)
r357614 added CTLFLAG_NEEDGIANT to make it easier to find nodes that are
still not MPSAFE (or already are but aren’t properly marked).
Use it in preparation for a general review of all nodes.

This is non-functional change that adds annotations to SYSCTL_NODE and
SYSCTL_PROC nodes using one of the soon-to-be-required flags.

Mark all obvious cases as MPSAFE.  All entries that haven't been marked
as MPSAFE before are by default marked as NEEDGIANT

6f490688f5 Improve support for switching to and from command polling mode in mlx4core.
Make sure the enter and leave polling routines can be called multiple times
with same setting. Ignore setting polling or event mode twice. This fixes a
deadlock during shutdown if polling mode was already selected.

c3191c2e2b Update the mlx4 core and mlx4en(4) modules towards Linux v4.9.
Background:
The coming ibcore update forces an update of mlx4ib(4) which in turn requires
an updated mlx4 core module. This also affects the mlx4en(4) module because
commonly used APIs are updated. This commit is a middle step updating the
mlx4 modules towards the new ibcore.

This change contains no major new features.

Changes in mlx4:
  a) Improved error handling when mlx4 PCI devices are
  detached inside VMs.
  b) Major update of codebase towards Linux 4.9.

Changes in mlx4ib(4):
  a) Minimal changes needed in order to compile using the
  updated mlx4 core APIs.

Changes in mlx4en(4):
  a) Update flow steering code in mlx4en to use new APIs for
  registering MAC addresses and IP addresses.
  b) Update all statistics counters to be 64-bit.
  c) Minimal changes needed in order to compile using the
  updated mlx4 core APIs.

fe3ca95c9d mlx4: Use the CQ quota for SRIOV when creating completion EQs
When creating EQs to handle CQ completion events for the PF or for
VFs, we create enough EQE entries to handle completions for the max
number of CQs that can use that EQ.

When SRIOV is activated, the max number of CQs a VF (or the PF) can
obtain is its CQ quota (determined by the Hypervisor resource
tracker).  Therefore, when creating an EQ, the number of EQE entries
that the VF should request for that EQ is the CQ quota value (and not
the total number of CQs available in the firmware).

Under SRIOV, the PF, also must use its CQ quota, because the resource
tracker also controls how many CQs the PF can obtain.

Using the firmware total CQs instead of the CQ quota when creating EQs
resulted wasting MTT entries, due to allocating more EQEs than were
needed.

64968e7065 Change mlx4 QP allocation scheme.
When using Blue-Flame, BF, the QPN overrides the VLAN, CV, and SV
fields in the WQE. Thus, BF may only be used for QPNs with bits 6,7
unset.

The current ethernet driver code reserves a TX QP range with 256b
alignment.

This is wrong because if there are more than 64 TX QPs in use, QPNs >=
base + 65 will have bits 6/7 set.

This problem is not specific for the Ethernet driver, any entity that
tries to reserve more than 64 BF-enabled QPs should fail. Also, using
ranges is not necessary here and is wasteful.

The new mechanism introduced here will support reservation for "Eth
QPs eligible for BF" for all drivers: bare-metal, multi-PF, and VFs
(when hypervisors support WC in VMs). The flow we use is:

1. In mlx4_en, allocate Tx QPs one by one instead of a range allocation,
   and request "BF enabled QPs" if BF is supported for the function

2. In the ALLOC_RES FW command, change param1 to:
a. param1[23:0]  - number of QPs
b. param1[31-24] - flags controlling QPs reservation

Bit 31 refers to Eth blueflame supported QPs. Those QPs must have bits
6 and 7 unset in order to be used in Ethernet.

Bits 24-30 of the flags are currently reserved.

When a function tries to allocate a QP, it states the required
attributes for this QP. Those attributes are considered "best-effort".
If an attribute, such as Ethernet BF enabled QP, is a must-have
attribute, the function has to check that attribute is supported
before trying to do the allocation.

In a lower layer of the code, mlx4_qp_reserve_range masks out the bits
which are unsupported. If SRIOV is used, the PF validates those
attributes and masks out unsupported attributes as well. In order to
notify VFs which attributes are supported, the VF uses QUERY_FUNC_CAP
command. This command's mailbox is filled by the PF, which notifies
which QP allocation attributes it supports.

2cebcdc7b6 Flexible and asymmetric allocation of EQs and MSI-X vectors for PF/VFs.
Previously, the mlx4 driver queried the firmware in order to get the
number of supported EQs. Under SRIOV, since this was done before the
driver notified the firmware how many VFs it actually needs, the
firmware had to take into account a worst case scenario and always
allocated four EQs per VF, where one was used for events while the
others were used for completions. Now, when the firmware supports the
asymmetric allocation scheme, denoted by exposing num_sys_eqs > 0 (-->
MLX4_DEV_CAP_FLAG2_SYS_EQS), we use the QUERY_FUNC command to query
the firmware before enabling SRIOV. Thus we can get more EQs and MSI-X
vectors per function. Moreover, when running in the new
firmware/driver mode, the limitation that the number of EQs should be
a power of two is lifted.

97549c34ec Move the ConnectX-3 and ConnectX-2 driver from sys/ofed into sys/dev/mlx4
like other PCI network drivers. The sys/ofed directory is now mainly
reserved for generic infiniband code, with exception of the mthca driver.

- Add new manual page, mlx4en(4), describing how to configure and load
mlx4en.

- All relevant driver C-files are now prefixed mlx4, mlx4_en and
mlx4_ib respectivly to avoid object filename collisions when compiling
the kernel. This also fixes an issue with proper dependency file
generation for the C-files in question.

- Device mlxen is now device mlx4en and depends on device mlx4, see
mlx4en(4). Only the network device name remains unchanged.

- The mlx4 and mlx4en modules are now built by default on i386 and
amd64 targets. Only building the mlx4ib module depends on
WITH_OFED=YES .
