Jason Evans 12fbf47cfb Fix some subtle bugs for posix_memalign() having to do with integer 
rounding and overflow.  Carefully document what the various overflow
tests actually detect.

The bugs mostly canceled out, such that the worst possible failure
cases resulted in non-fatal over-allocations.
